A circular slab penetration greater than four inches in diameter is wrapped by a one-half inch flexible closed-cell foam plank extending the full depth of the slab, finished with sealant. Two number three hooked reinforcing bars are placed in the top and bottom with one-inch clearance from the top and two-inch clearance from the bottom on one side, and one and one-half inch clearance from the top and two and one-half inch clearance from the bottom on the opposite side. A typical one-inch clearance is maintained around the penetration assembly with a one-foot six-inch typical dimension to the outside reinforcing hook.
