This assembly consists of a headed anchor rod secured with a round plate washer. The washer is tack-welded to the rod head in the shop at three locations. The embedment depth is measured from the top of the plate washer to the top of the rod head. The assembly is designated as Type 6, which is equivalent to Type 3 unless noted otherwise.
