A tie-back anchor and horizontal lifeline anchor connect to a supporting steel beam beneath a concrete slab on a metal deck. The connection utilizes a five-eighths-inch plate on each side, welded to the beam web with half-inch and five-sixteenths-inch fillet welds and complete joint penetration welds at the top and bottom flanges. The plate thickness equals the beam flange thickness plus one-eighth of an inch and matches the beam flange width, though plates are not required where bottom flanges align. Bolts connect the assembly per plan with three-inch on-center spacing and one-and-a-half-inch edge distances.
