An anchor rod assembly features a steel base plate with oversized holes resting on a layer of non-shrink grout above a concrete support. Leveling nuts are positioned beneath the base plate, while two standard nuts and a plate washer secure the top of the plate. The threaded anchor rod extends downward with a minimum embedment length per schedule and includes two standard nuts and one standard washer at the bottom end. Circular or square washers are acceptable for the installation.
