A circular slab penetration greater than four inches in diameter is isolated with a one-half-inch flexible closed-cell foam plank spanning the full depth of the slab, finished with sealant. Two number three hooked bars are placed at the top and bottom with one inch of clearance typically on one side and one and one-half inches of clearance from the top with two and one-half inches from the bottom on the other side. The penetration features a one-inch typical clearance surrounding the perimeter sleeve. Reinforcement spacing extends one foot six inches typically from the edge of the opening.
