Clearance requirements and reinforcement criteria apply to round or square slab penetrations near post-tensioned tendon anchorages. A schedule 40 steel pipe insert is required whenever a slab penetration falls within a forty-five degree bearing cone of the tendon anchorage. Penetrations located farther than 1.5 times the diameter or width, and not less than eighteen inches from the anchor, do not require a steel pipe insert. Any penetration with dimensions exceeding twelve inches requires additional trim bars.
