Concrete floor slab openings are reinforced with number four bars placed around the perimeter and diagonally at the corners. Diagonal bars extend two feet typically from each corner of the opening with a one-and-a-half-inch typical clearance. Only diagonal bars are required for openings smaller than one foot by one foot. Unshown openings larger than one foot square and closely spaced multiple holes require submission to the structural engineer for review.
