A pipe penetrates a concrete slab on grade through a pipe sleeve spanning the full thickness of the concrete. The assembly maintains a minimum one-half inch and maximum one-inch clearance around the pipe, increased to one-and-one-half inches of clearance for gas pipes. The annular space is packed with a minimum three-inch depth of fiberglass pipe insulation and sealed at the surface with a minimum one-half-inch depth of silicone sealant.
