An insulated or uninsulated pipe penetrates a concrete floor slab through a pipe sleeve secured with a minimum of three poured-in-place anchor bolts. The annular space between the sleeve and the pipe is sealed with backing material as required and finished with sealant on both sides. A vapor barrier is incorporated into the insulated assembly, and a riser clamp is provided where applicable.
