An isolation damper assembly includes a damper actuator provided with the damper unit, configured to fail in place upon loss of signal or power. Two mechanical position switches are installed to monitor the damper state, with one switch opening when the damper is closed and the other closing when the damper is closed. Damper status signals are routed to the life safety system via dedicated wiring connections. The assembly is located at level six in the mechanical well.
