An eighteen-inch square by three-eighths-inch thick stainless steel plate fits over a twelve-inch pipe opening. Four five-eighths-inch vertical slots accommodate one-half-inch diameter by six-inch stainless steel anchor bolts typically positioned at fifteen and sixteen inches on center. A three-and-five-eighths-inch orifice is set at the bottom of the plate, with note instructions indicating the plate length may need enlargement in the field to cover pipes installed on angles.
