A stair guard rail assembly mounts to a steel stair stringer with concrete-filled tread pans. A vertical support extension uses a welded flat bar tab that is through-bolted to the stair stringer web. The handrail maintains a 42-inch minimum height with a maximum 4 1/2-inch projection into the tread zone, and pickets or balusters comply with the 4-inch sphere rule.
