Anchor bolts are inserted into sleeves that penetrate a concrete slab on grade, with the sleeve extending two inches into the concrete below the top surface. The assembly includes hooked anchor rods positioned inside protective sleeves embedded in the concrete floor slab. A typical detail reference bubble points to the embedded sleeve assembly.
