A galvanized steel pre-manufactured fall arrest anchor is anchored to a structural steel member using a steel plate with sizing and bolt lengths determined by the anchor manufacturer. Four three-quarter-inch diameter stainless steel bolts with lock washers and nuts secure the base, featuring deformed threads top and bottom per OSHA guidelines with a minimum of two exposed threads typically at each bolt. The anchor post pierces the multi-layer roof assembly comprising rigid roof insulation, a cover board, and membrane roofing. An eight-inch preformed vent flashing fits over the post and is secured with a stainless steel clamp and roof manufacturer approved sealant at the top, while the base of the flashing is attached to the membrane roofing with at least four membrane attachments and sealed with a hot air weld.
