A nominal three-inch diameter or smaller steel, iron, steel pipe, or steel metallic conduit passes through a minimum five-inch thick reinforced lightweight or normal weight concrete floor assembly. The annular space is packed with nominal three-inch thick, five pounds per cubic foot density mineral wool batt insulation, sized to fit the pipe or conduit. The pipe covering is secured with steel wire ties on each side of the floor assembly and extends twelve inches long to maintain the two-hour F and T ratings. Elastomeric sealing plugs are installed at the surfaces to accommodate the core-drilled opening and penetrate around the pipe or conduit.
