An as-cast formed concrete slab supports a scheduled door frame and door with an integrated threshold. The threshold transitions to a maximum half-inch height above a sealed concrete or resinous floor on one side. The opposite side abuts carpet tile or a walk-off mat as specified on the finish plan.
